What causes chronic pain when you walk and stand?

The problem is structural and one of angles. It stems from a frontal plane (front view) curvature in the Tibia (lower leg). It appears to be present in most all humans from birth.

This angular deviation is documented visually as Tibial Varum or Bow Legs.

This curvature in the lower leg can be measured non-weight bearing, between the horizontal axis of the knee, and the horizontal axis of the ankle.

Functionally during weight bearing, the body must compensates for this 20 degree frontal plane, or varus deformity present in the tibia. Some people plantar-flex the first metatarsal, forming a high arch, and stabilizing the foot on the ground. Other people flatten the arch and pronate the back of the foot or Sub-Taylor Joint, bringing the bottom of the foot down to the horizontal plane of support (floor). Once the foot is secured on a stable surface, a balanced gait can be maintained and walking and standing is possible, even with a chronic biomechanical “price”.

As we stand and walk, attempts by the body to compensate for this angular deviation of the tibia, ultimately fail and biomechanical dysfunction is inevitable.

This varus twist in the lower leg, results in less than optimal foot function as we walk and stand, and is the cause of many chronic medical conditions in the feet, ankles, knees, hips, and back. 

For more than 30 years, thousands of clinical cases using treatment products that compensate for this curvature in the tibia, have documented the relationship between THETA (quantified angular wedges ) and chronic pain caused by walking and standing.

Tibial Varum can be easly visualized in all of these models posed in the frontal plane. The curvature is often more obvious in one leg, as even minor rotations from the frontal plane will conceal the deformity.

Many self proclaimed ”biomechanical experts” continue to deny the existence and important relationship these angles have with human function during gait.

Orthotics use wedges to compensate for this Frontal Plane deformity.
