When the neuroma first begins, there may only be a loss of sensation between the 2nd and 3rd, or 3rd and 4th toes. Most patients don’t worry or are not even aware of the problem at this point. With time the pain in this same region begins and usually results in intermittent, sharp electrical sensations or even a constant burning just between the metatarsal heads.
.When the condition first begins, it can usually be relieved by avoiding shoes that place additional weight or compression on the metatarsal area of the foot. With time and additional activity, the pain increases and is often present during all weight bearing activities in all shoe types.

In the early stages, pain may be relieved by avoiding higher heeled shoes, or by wearing soft insoles and supportive athletic or walking shoes. As the nerve becomes more irritated, swollen and inflamed even high doses of anti-inflammatories generate little to no relief of the symptoms. Cortisone injections are usually only temporary and multiple alcohol injections are of little benefit. Neuroma surgery is at best a “crap shoot;” as even when the result is “good,” reoccurrence or generation of a new neuroma in the adjacent inter-space or other foot usually occurs.

For the most part, it is commonly accepted that the cause of the problem is excess pressure on the inter-metatarsal nerve between the 2nd and 3rd, or 3rd and 4th metatarsals. It is believed that collapse of the long arch of the foot results in a breakdown of the metatarsal arch and places excessive pressure on the involved nerves. Although this condition generally begins with only one nerve it often progresses over time to involve nerves in both inter-spaces in both feet. The pain associated with weight bearing activity frequently reaches a point were the patient pleads to have their neuromas removed surgically. Although this is a tempting option, it rarely results in a good result, as not only can the nerve regrow, this time in scar tissue from the surgery, but often it fails to resolve much if any of the pain. Complications following the surgery include stump neuromas, new neuromas in the adjacent inter-space or opposite foot, heavy painful scar tissue, and other common surgical problems. Successful treatment requires something that will decrease the pressure on the involved nerves on a permanent basis.

Simple treatments that use pads to support the metatarsal arch or spread the metatarsals so as to reduce some of the pressure on the nerve may be helpful in the early stages of the problem. Even avoidance of higher heeled shoes sometimes helps initially. Eventually, if the patient remains active, more aggressive attempts to reduce the pressure on the nerve are required to get any help at all. Custom orthotics that attempt to restore the longitudinal arch , maintain the metatarsal arch , and relieve the pressure, irritation, inflammation, swelling and pain have been reported as effective treatments for many years.
Many foot specialists will suggest that even those custom orthotics, when worn during all weight bearing periods, fall short of stopping the progression of this painful condition. Nerve pain is difficult to treat in any part of the human body but on the ball of the foot it can be a nightmare. When orthotics fail to adequately relieve the pain, it is usually because they fail to relieve sufficient pressure from the nerve.

Neuromas are probably one of the more difficult conditions that I treat. Recovery from nerve damage involves a reawakening process that can have set backs and times where you wonder if the treatment will work.
